President Donald Trump has announced that the federal government will send a one-time $90 payment to more than 20 million Americans enrolled in Medicare, to help offset the cost of their monthly Part B premiums. Trump made the announcement on social media late on Friday, October 2, according to Reuters.

What was announced. The money will come from the Medicare Improvement Fund, which Congress created in 2008 and funded with $2 billion for improvements to the traditional fee-for-service Medicare program. A White House fact sheet says the fund has never been used before and describes Trump as the first president to use it to directly lower costs for seniors. Trump has called it a “slush fund” in his post. In the same post, he said the checks would be “nearly $100,” and the White House later specified $90 per person.

Who qualifies. Per the White House, most people enrolled in Part B will receive the payment. It will not go to people whose Part B premiums are paid by Medicaid, or to higher-income beneficiaries who pay the income-related monthly adjustment amount (IRMAA), according to local news reports citing the White House. The Associated Press, via US News, adds that recipients must be living in the United States and that people enrolled in Medicare Advantage, a private alternative to traditional Medicare, are not eligible. KFF estimates that more than half of eligible beneficiaries are in Medicare Advantage, which helps explain why the figure is about 20 million.

When and how. Most recipients will get a $90 direct deposit in early October. People without direct deposit on file will receive a paper check mailed to the address registered with Medicare, the White House said. Those who want to check on a payment can call the Social Security Administration at 1-800-772-1213, according to WKRC.

How big is it? The standard Part B premium in 2026 is $202.90 a month, according to the Centers for Medicare & Medicaid Services, so $90 is less than half of one month’s premium. Our calculation: 20 million payments of $90 total about $1.8 billion, which fits within the fund’s $2 billion.

The political context. The announcement comes about a month before the November 3 midterm elections that will decide control of Congress. Reuters describes it as the latest in a series of financial promises Trump has made to voters ahead of the vote. The administration is also sending $500 payments to nearly 1 million Americans who were overcharged for Affordable Care Act plans, a step strategists say could potentially influence some races. Separately, Trump has proposed a $5,000 “dividend” for American adults, which he has tied to Republicans keeping control of Congress. The AP notes that Trump has played down affordability concerns by saying the economy is booming.

Two ways to read it. The White House presents the payments as a direct effort to lower costs for seniors, alongside Trump’s “most favored nation” drug pricing deals. Critics and political strategists point to the timing and note that it is a one-time payment rather than a change to premiums. Both points can be true at once: the money is real for those who receive it, and it arrives during a campaign.

What to watch next: whether payments arrive on schedule, whether legal or congressional questions arise about using the Medicare Improvement Fund this way, and how voters respond.

Tip for readers: officials do not usually call to ask for bank details. If someone does, hang up and call the Social Security Administration directly.
